Interested in joining a program?

Step 1:  Fill out our brief Client Inquiry Form

Step 2:  Download and print the Client Enrollment Application, which includes the the Client Medical History and Physician's Release which must be completed by your doctor.

Step 3:  Once you've contacted SMILES using the Inquiry Form, our Program Coordinator will reach out to schedule a cost-free evaluation at the SMILES facility.  During the evaluation, you will:

  • Receive a full facility tour
  • Discuss program options and go into dept on our progressive 8-week curriculum
  • Establish individual goals to be addressed in programing
  • Discuss available class times

Step 4:  Send the Client Enrollment Application to the Program Coordinator in one of the following ways:
